New Service Request

Welcome to the Breezy Hill Water & Sewer Co., Inc. Community. 

  • Please come by our office and complete the New Tap form, or you can print out the New Tap & Transfer Water form below. All supporting documentation must be scanned in and emailed to us, or brought into our office.
  • Complete this form (make sure it is signed),along with a scanned copy of your driver's license and email it to: info@bhws.org
  • Driver’s License of Account Applicant (Address not Required to Match).
  • If Owning Property – Closing Papers, Deed, HUD Statement, Property Tax Record or Similar with Name Matching Driver’s License and Property Address Listed on Document.
  • If Renting Property – Some Form of Documentation Serving as a Renter’s Agreement or Lease Agreement. If No Such Documentation is Available, Must Have Two Forms of Proof (another utility bill, mail received at address, renter insurance, etc.)
  • Once these documents are recieved, we will set up your new water service.
  • You will receive a call from our customer service team to assist you with paying the deposit over the phone . We look forward to serving your water needs.
